mg or ml vial product charges shipping & handling cost. We accept
checks and cashier's checks in U.S. dollars and drawn on a U.S.
bank. All international orders must be pre-paid by check or
wire transfer in USD. Payments Terms Net 30 is available upon

| Direct Purchase Order Requirements:
All purchase orders must include the following information:
PO number, PO date, and name of authorized purchasing agent.
Complete ¡°Bill To¡± information, including AP contact, email
address, phone and fax numbers. Complete ¡°Ship To¡± information,
including email address and phone number. |

Shipping Information:
Delivery is from five to fifteen days from the date of your order. Kollodis BioSciences will
provide estimated shipment dates, but cannot be responsible for delays in delivery due
to events beyond our control. Products are shipped via US Mail, or can be expedited via
FedEx upon request
